Medical Questionnaire

Filling up this medical questionnaire is mandatory for every diver. This questionnaire is developed by Scuba Schools International (SSI) and is in accordance with global practices in the scuba diving industry.

You may also refer to this questionnaire to determine your eligibility to dive

or to have a conversation with your doctor.

We can arrange for you to meet a doctor in Pondicherry who would examine you and sign-off on this medical questionnaire if she/he is satisfied. Please note, this may require a few hours before your first day of diving.
